ZGODA NA PUBLIKACJĘ WIZERUNKU
Oświadczam, że zapoznałem się z Regulaminem wydarzeń, organizowanych przez Goyki 3 Art Inkubator z siedzibą w Sopocie i wyrażam zgodę na publikację, wykorzystanie i rozpowszechnianie, w tym w szczególności umieszczanie i prezentowanie, przez Organizatora lub podmioty z nimi współpracujące, mojego wizerunku.
Jednocześnie oświadczam, iż ww. zgoda wyrażona powyżej jest nieograniczona w czasie i terytorialnie oraz dotyczy również wykorzystania i rozpowszechniania wizerunku w dowolnym utworze oraz w innych materiałach nienoszących cech utworu, z zastrzeżeniem, że wykorzystanie i rozpowszechnianie będzie związane z realizacją w celu informowania o działalności realizowanej przez Organizatora i będzie zgodne z dobrymi obyczajami.
Wyrażam zgodę na wykorzystanie mojego wizerunku w szczególności poprzez jego:
publikację na stronie internetowej Organizatora, na profilach Organizatora w mediach społecznościowych i jego udostępnianie użytkownikom Internetu w inny sposób;
zwielokrotnianie, utrwalanie, sporządzanie cyfrowego zapisu wizerunku, wprowadzanie utrwalonego wizerunku do pamięci komputera;
wprowadzanie utrwalonego wizerunku do baz danych Organizatora;
w zakresie rozpowszechniania wizerunku w sposób inny niż określony powyżej – publiczne wykonanie, wstawienie, wyświetlenie, odtworzenie oraz nadawanie i reemitowanie, a także publiczne udostępnianie utrwalonego wizerunku w taki sposób, aby każdy mógł mieć do niego dostęp w miejscu i w czasie przez siebie wybranym, w szczególności za pośrednictwem telewizji, Internetu lub innych sieci komputerowych, sieci telefonii komórkowych oraz w jakikolwiek inny sposób, w tym przy pomocy przekazu telekomunikacyjnego lub satelitarnego.
Oświadczam, iż wyrażam zgodę na przetwarzanie moich danych osobowych dla celów niniejszego oświadczenia.
Appendix to the Rules and Regulations of Events organised by Goyki 3 Art Inkubator
VISUAL LIKENESS RELEASE FORM
I hereby declare that I have acquainted myself with the Rules and Regulations of the events organised by Goyki 3 Art Inkubator, based in Sopot, and I consent to the publication, use and dissemination, including in particular uploading and displaying my visual likeness by the Organiser or entities cooperating with them.
I further declare that the aforementioned consent is unlimited in time and territory and also applies to the use and dissemination of the likeness in any work and in other materials that do not have the characteristics of a work, subject to the fact that the use and dissemination will be related to the implementation for the purpose of informing about the activities carried out by the Organizer and will be in accordance with good morals.
I consent to the use of my likeness in particular through its:
publication on the Organiser’s website, on the Organiser’s social media profiles and its sharing with Internet users in any other way;
reproduction, recording, making a digital record of the image, entering the recorded image into computer memory;
introduction of the recorded image to the Organiser’s databases;
with regard to the dissemination of the image in a manner other than specified above – public rendition, exhibition, display, reproduction, as well as broadcasting and re-broadcasting and making the captured likeness available to the public in such a way that everyone can have access to it at a place and time of their own choosing, in particular via television, the Internet or other computer networks, cellular networks and in any other manner, including by means of telecommunication or satellite transmission.
I hereby give my consent to the processing of my personal data for the purposes of this declaration.
